Assistant Instructor students are required to complete the following forms before the start of the course (unless these have previously been completed)

  • Liability Release and Assumption of Risk Agreement
  • Divers Medical Form
  • DiveSmart Agreement
  • NovoScuba Professional Membership Agreement
